To use Patient Access and receive full access we will need to see photographic identification. You may register without a letter but will be required to provide identification before full access will be given. Please visit our reception to complete your registration. A passport or photo card driving licence is preferable. However, if you do not have either, a utility bill or bank statement dated within the last six months will be accepted.
If you are housebound and unable to visit the practice to provide your identification please contact Sandra Hawkins at the practice who will advise you further.
